CANCELLATIONS
Please call our office as soon as possible if you are unable to keep an appointment. If you are going to be late by 15 minutes or more, please let us know. Failure to cancel an appointment prevents other patients from being seen and increases cost of care. If you have more than two “no shows”, you will be charged $20 which will be due prior to your next appointment.
PRESCRIPTION RENEWALS
Please give us 48 hours to refill your medication if you will run out before your next appointment. Please call your pharmacy to find out if you still have any refills left. You will be asked to provide:
- Name of the medication
- Dosage
- Directions for use
FEES
Our contract with your insurance company requires us to collect your copay at the time of visit.
We ask that you pay your copay and charges not covered by your insurance at the time of visit. Your prompt payment decreases administrative costs and results in lower cost of healthcare.
If you have financial reasons which prevent you from paying at the time of visit, please let us know. We can design an affordable payment schedule for you.
Accounts more than 90 days past due may be sent to an outside agency for collection.
